Recovering from an injury, hospital stay, or surgical procedure can be difficult at any age, particularly when daily activities become a challenge.  At Lutheran Crossing, we help our residents regain strength and return to their active life as soon as possible.  Thanks to our team of physical, occupational and speech therapists we work with every aspect of a person’s life, so transitioning back home is easy.

Rehabilitation is available for both residents who live at Lutheran Crossing and for those who live in the community and require outpatient rehabilitation. Our highly trained therapists develop individualized care plans and employ state-of-the-art equipment and evidence-based rehabilitative programs. Medicare and most insurance plans are accepted for both inpatient and outpatient services.

Inpatient Rehabilitation

Lutheran Crossings at Moorestown offers short- and long-term subacute inpatient medical rehabilitation in a comfortable, home-like setting. Treatment may include a combination of physical, occupational and/or speech therapy, typically lasting one to three hours per day.

Our therapists have been trained in specialized programs that are based on research. With specific diagnoses and individual evaluation these programs are used to maximize the patient or resident’s participation in daily tasks and functional skills. Some of our programs include:

Dementia Pathways Program
Through this program, therapists work with specific stages and levels of Alzheimer’s and Dementia by developing individualized plans that identify the best activities and strategies to encourage self-care and daily activities. This program also provides information to the caregivers and staff to help better understand patient needs and behaviors associated with the diagnosis.

HeartWorks Program
HeartWorks is an interdisciplinary program which provides a guideline for the care and treatment of our residents with varying cardiac diagnoses, including chronic heart failure, coronary artery disease, acute myocardial infarction, postoperative coronary artery bypass grafting, and conduction abnormalities. The program provides our therapists with diagnosis-specific recommendations for each patient with the goal to improve cardiac function, cardiac health and overall quality of life.

BreathWorks Program
The BreathWorks program is designed to identify residents who may be at risk for re-hospitalization secondary to pulmonary conditions, such as COPD and pneumonia. These patients may benefit from a comprehensive pulmonary program, including education, optimization of medical therapy, exercise training, psychosocial support, controlled breathing techniques, and continuing care programs, in order to reduce the effects of acute and chronic pulmonary symptoms. Patients are evaluated and assessed using standardized testing procedures, clinical tests and measures, and patient self-reporting tools. Our therapists develop a specific and individualized plan of care that is based on the patient’s severity level, clinical signs and symptoms, and goals.

All rehabilitation services are performed under the careful supervision of our team of physical, occupational and speech therapists, registered dieticians, social workers, nurses, psychologists and support staff. Team members work together to understand your pre-injury level of activity and then restore your ability to function independently.

Patients who do not progress to a level that allows them to return home comfortably may consider transitioning into our on-site Assisted Living or comprehensive skilled nursing program. This continuum-of-care option simplifies the patient’s transition, alleviating the stress of relocating.

Outpatient Rehabilitation

Open to anyone in need of these services within the greater Moorestown community, our outpatient rehabilitation services include physical, occupational and speech therapy.  Our rehabilitation staff are all highly trained and experienced in working with adults who are recovering from an injury or illness.  Whether you want to get back to the tennis court or simply walk without pain, our therapists work with you to improve balance, flexibility, strength and endurance.

Lutheran Crossings outpatient rehabilitation services are open to residents and members of the outside community. Certified by the State of New Jersey, we specialize in adults and seniors.

Bring your medical prescription. We’ll work with your insurance and make sure you enjoy a simple and easy process.
