Assisted Living

Making the decision to choose assisted living is an exciting one. However, it can also be a very daunting process. Hopefully, you (or your loved one) have properly prepared financially for this endeavor, and you have already gathered all the necessary paperwork regarding your financial portfolio.

If you are on the other side of the coin, and have not done anything to prepare and you are not even sure you can afford assisted living, you can use this financial calculator to calculate your current monthly living expenses. Next, gather all the information you can about your income, investments, insurance, and savings. This information is then presented to the admissions team, along with your application and suite selection, to help determine if you are financially approved as a new resident of Lutheran Crossings Enhanced Living at Moorestown. Admission is also based upon your clinical assessment and our suite availability.

Upon financial approval, we’ll provide you with an Admissions Packet, which includes paperwork that will need to be completed and returned. Some forms will need to be completed and signed by your physician. Please be sure to have all health history information gathered in order to complete the Medical and Social History form. Our Resident Services Director will contact you to schedule a clinical assessment, usually conducted at your home.

Once all paperwork is received and your clinical assessment is conducted, our admissions team will sit with you to review your Prescription for Wellness, assign a move-in date, and have you review and sign the admissions agreement.

After that – it’s Welcome Home!


Skilled Nursing Care & Rehabilitation

When it is time to consider 24-hour skilled nursing care, it is sometimes difficult getting everyone in the family to agree. So many emotions take over. Whether it is guilt for relinquishing the caregiving role, disagreement among family members, or sadness to see a loved one incapable of caring for themselves anymore, the important thing to remember is that you are not alone. There are plenty of resources and senior care options that we can provide to you and your family to make the best decision for your loved one.

If you or a loved one are suddenly hospitalized and in need of sub-acute rehabilitation, your hospital social worker will help you through the process of locating a facility. If you prefer Lutheran Crossings at Moorestown be your first choice among your options, please be sure to communicate that to your hospital care team. The coordination of information is managed directly between the hospital and our admissions office. However, feel free to contact our admissions department directly to come in, tour, and discuss your loved ones needs.

When determining admission, first considerations include the resident’s medical needs, insurance coverage, and bed availability. If your loved one is in need of long term skilled nursing care, an admission application must be completed. If everything is reviewed and approved, arrangements are made for the resident’s arrival, and for their care throughout their stay.

For various reasons, we may not be able to provide the care your loved one requires. If that should be the case, our admission team will be happy to assist you by providing a list of local alternative providers.
